What to Bring to a Florida Government Office

A practical checklist of documents to bring for a driver license, vehicle registration or title, voter registration and county health visits.

For every visit

Driver license or ID card

First-time Florida applicants and anyone applying for a REAL ID-compliant card are generally asked to show proof of identity, proof of Social Security number, and two proofs of where they live. Examples: a passport or birth certificate; a Social Security card or W-2; and recent bills, a lease, bank statement or similar mail that shows your name and Florida address. If your name differs from your birth certificate, bring the legal name-change documents (marriage certificate, court order). FLHSMV publishes the exact list by residency status, so check it first.

Vehicle registration or title

Voter registration

You can register online or at your county Supervisor of Elections. You will need your Florida driver license or ID number, or the last four digits of your Social Security number. Check your county office for early voting sites and deadlines.

County health department

Bring photo ID, your insurance card if you have one, and immunization records for vaccine visits. Services and fees differ by clinic, so call ahead.
